Only the vehicle owner may request a corrected title.
Any other owner listed on the title must appear with you and bring their identification. (If an owner cannot appear, they may complete and sign an Appointment of Agent form and provide a photocopy of their driver’s license.)
- Your vehicle title
- Your vehicle registration
- Your corrected driver’s license or an official document verifying your name change
- $15 title correction fee
- Debit or credit card (additional fees apply), cash, check, or money order for payment

- Lien termination statement from original lienholder or original lienholder sign-off on title
- The address and name of the lienholder being added to the title. You do not need permission from a first lienholder to add a second lienholder.
- The lien termination statement from original lienholder or original lienholder sign-off on title if removing a lienholder
- $15 title correction fee
- $1 lien filing fee (No fee for removing a lienholder)
